①具有獨立承擔(dān)民事責(zé)任的能力：提供法人或其他組織的營業(yè)執(zhí)照等證明文件；提供法定代表人身份證及法定代表人身份證明（或委托代理人身份證及授權(quán)委托書）；

②具有良好的商業(yè)信譽(yù)和健全的財務(wù)會計制度：提供'Times New Roman',serif\">2024年度經(jīng)審計的財務(wù)報告或'Times New Roman',serif\">2025年'Times New Roman',serif\">1月至今基本

③具有履行合同所必需的設(shè)備和專業(yè)技術(shù)能力：提供相關(guān)證明材料或出具承諾函（承諾函格式自擬）；

④具有依法繳納稅收和社會保障資金的良好記錄：提供'Times New Roman',serif\">2025年至今任意連續(xù)三個月依法繳納稅收和社會保障資金的證明材料或出具承諾函（承諾函格式自擬）；

⑤參加本次采購活動前三年內(nèi)，在經(jīng)營活動中沒有違法違規(guī)記錄：提供書面聲明（格式詳見采購文件）；

⑥法律、行政法規(guī)規(guī)定的其他條件：投標(biāo)人須承諾在″信用中國”網(wǎng)站（'Times New Roman',serif\"> ）、中國 （'Times New Roman',serif\"> ）等渠道查詢中未被列入失信被執(zhí)行人名單、重大稅收違法失信主體、政府采購嚴(yán)重違法失信行為記錄名單中，如被列入失信被執(zhí)行人、重大稅收違法失信主體、政府采購嚴(yán)重違法失信行為記錄名單中的投標(biāo)人取消其投標(biāo)資格，并承擔(dān)由此造成的一切法律責(zé)任及后果。
